All students enrolled in the preschool (new students), KG, 2nd Grade, 6th Grade, 9th Grade, 12th Grade and Students entering a State of Illinois school for the first time are required by law to submit the following health documents. Please submit documents by the first day of school. I Pre-K1 and Pre-K2 (new students) are required by law to submit the following health documents. 1. Students entering a State of Illinois school for the first time. New students enrolling in an Illinois school for the first time are required to provide a physical examination (with all up-to-date immunization) and eye exam regardless of grade.
