This is to remind all students entering Pre-K1, Pre-K2 (new students), Kindergarten, 2nd Grade, 6th Grade, 9th Grade, and 12th Grade to have all the necessary NEW health documents paperwork (see email sent on 07/25/23 for details) ready to be submitted by the first day of school.

Students entering from out-of-state also need to have all health documents (physical, immunization, and eye) on the Illinois health form.
